HIPAA Notice of Privacy Practices

The County of Sonoma (Plan Sponsor) will protect the privacy of your protected health information (PHI). We also require subcontractors or agents to protect your PHI. PHI is health information that includes your name, Social Security Number, or other information that reveals who you are. You may generally see and receive copies of your PHI, correct or update your PHI, and ask us for an accounting of certain disclosures of your PHI.

We may use or disclose your PHI for health plan operations purposes, including paying claims, health research and measuring the quality of care and services. We are sometimes required by law to give PHI to government agencies or in judicial actions. We will not use or disclose your PHI for any other purpose without your (or your representative’s) written authorization, except as described in the County of Sonoma Privacy Manual and each plan’s Notice of Privacy Practices. Giving us authorization is at your discretion.

This is only a brief summary of some of our key privacy practices. The County of Sonoma Privacy Manual describing the County’s policies and procedures for preserving the confidentiality of medical records and other PHI is available and will be furnished to you upon request. You can request a copy of the policy from the County Compliance/Privacy Officer below. Each health plan’s Notice of Privacy Practices is contained within the Evidence of Coverage or Summary Plan Description of each plan. They can be obtained at the following link:
